GO PT is a participating provider with many insurance companies/networks and we participate with almost all employer sponsored health plans in our geographical area. Please contact our office with any questions regarding our participation with your particular insurance plan.

Although you may "self refer" for physical therapy treatment, most all insurance companies require a prescription from a physician in order to utilize your physical therapy benefit. In addition, depending on your particular insurance plan, you may also need to obtain an "Insurance Referral Form" from your Primary Care Physician, PCP. Our office staff will verify your insurance coverage on or before your first visit.

We will bill your insurance company for you, but we require co-payments and deductibles to be paid at the time of service.

Patients that carry health care insurance should remember that most plans require a co-payment for each visit. Consequently, it is your responsibility, as defined by your policy, to make these co-payments. Also important is that you are responsible for any and all supplies, such as braces, exercise equipment, orthotics, Jobst Stockings, etc., which are provided to you and not covered by your particular plan.

• Your name
• The referring physician's name, telephone and fax number
• The Injured Workers' name, home and work telephone numbers, social security number, and date of birth
• The Injured Workers' insurance and employer information
• Diagnosis and frequency and duration of therapy requested
• Date of injury
• Claim number
• Case manager's name, telephone and fax number

You may also want to fax the referral to our office at (301) 334-6277 to expedite your care
as most Case Managers require a faxed copy of your physician's order prior to authorizing physical therapy. GO PT looks forward to assisting you with your Injured Workers' claim.
